➊你叫什么名字?
什么. Adj "what" can be used to raise a question with a question mark in the end.
    这是什么?那是什么衣服? Zhè shì shénme? Nà shì shénme yīfu? - What's this? What dress is that?
    水是什么颜色? Shuǐ shì shénme yánsè? - What color is water?

➋你是老师吗?
subject + 是 shi + object. "Subj... to be..." is a very simple structure. Note it is same for all tenses. e.g.
     你哥哥 Nǐ shì gēge - You are elder brother.

是,是的/不,不是. This is Chinese way to answer "yes" or "no". e.g.
    你是他的爸爸吗? Nǐ shì tā de bàba ma? - You are his father.
    是,是的/不,不是 Shì, shì de/bù, bùshì - Yes, I am./No, I am not.

➌你是中国人吗?
statement + 吗? to raise a question. This is Chinese way to directly raise a question related to the statement. e.g.
    你是他的爸爸 Nǐ shì tā de bàba - You are his father.
    你是他的爸爸 Nǐ shì tā de bàba ma? - Are you his father?

➍我是美国人。
location name + 人: person from that location. In Chinese, when you want to express "I am from ...(a location name such as city)", just use "... + 人". Location name can be a country, a city or a geographic name.
    你是哪儿人 Nǐ shì nǎ'er rén? - Where are you from?
    我是中国人 Wǒ shì Zhōngguó rén. - I am Chinese (I am from China).
    我是上海人 Wǒ shì Shànghǎi rén. - I am from Shanghai.
    我是北方人 Wǒ shì běifāng rén. - I am from northern part (of the country).

➎她是我的汉语老师,
的 de. "Noun/pronoun + 的" is like "of + noun/pronoun" expressing possession or attribute. Sometimes, can be dropped. e.g.
    我妈妈 wǒ de māma - my mother
    你哥哥 nǐ de gēge - your elder brother
    我爸爸=我爸爸 wǒ de bàba - my father
    我爸爸妈妈是我奶奶 Wǒ bàba de māma shì wǒ nǎinai - My father's mother is my grandma.

➏她叫李月。
我叫...(name) to introduce oneself. “My name is ...". This is a way to say full name with surname first, then given name.
    我谢家家 Wǒ jiào Xiè Jiājiā - My name is Xie Jiajia.

➐我是美国人。你呢?
呢 (ne): particle.It has several applications when it is used in the end of sentence.
(1) indicating that a previously asked question is to be applied to the preceding word, like "What about ...?", "And ...?"
    我睡觉了,你 Wǒ shuìjiàole, nǐ ne? - I go to sleep. How about you?
(2) inquiring about location, like "Where is ...?"
    我的书 Wǒ de shū ne? - Where is my book?
(3) signaling a pause, to emphasize the preceding words and allow the listener time to take them on board, like "ok?", "are you with me?"
    你想踢足球,还是打篮球
    Nǐ xiǎng tī zúqiú ne, háishì dǎ lánqiú ne? - Will you play soccer or basketball?
